Dermal Fillers

Dr Georghy has been injecting dermal fillers since 1995.

Excessive sweating (Hyperhidrosis)

Hyperhidrosis is a condition which causes excessive sweating. According to research nearly 4 percent of the world’s population suffer from symptoms of hyperhidrosis. Sweaty palms, hands, arms, face and feet can cause embarrassment and social issues. Day-to-day life may become difficult and mingling closely with others an uphill battle. However, hyperhidrosis is a treatable condition and does not have to end in social isolation.

For those suffering hyperhidrosis, Dr Joseph Georghy offers two treatments, both designed to alleviate the very real and often embarrassing symptoms of heavy sweating. Although a temporary solution, with results up to a year, injections with Botulinum toxin type A under the arms, on the palms and soles of the feet can inhibit the release of a chemical that signals the sweat glands to activate. Dr Georghy also offers a surgical solution, known as LipoScraping, which involves superficial liposuction to remove some of the sweat glands.

Hyperhidrosis can be divided into two main categories, namely focal hyperhidrosis or primary hyperhidrosis and generalized hyperhidrosis or secondary hyperhidrosis:

  • Focal Hyperhidrosis or Primary Hyperhidrosis. This type of hyperhidrosis causes excessive sweating in the arm area (including underarms), hands, feet and face. Focal hyperhidrosis is a result of a glitch in the nervous system. The sweat glands are stimulated to produce an excess of sweat.
  • Generalized Hyperhidrosis or Secondary Hyperhidrosis. Generalized hyperhidrosis causes the entire body to break out in excessive sweating. It's the result of certain health conditions such as metabolic disorders, neurological disorders and menopause.

Botulinum Toxinum and Iontophoresis are two of the main treatments for hyperhidrosis. Surgery and medication are also options. Consider seeing a doctor to see which treatment fits you best.

Possible side effects

Worldwide, Botulinum Toxinum (BT) has been used since 1989 to treat a variety of conditions, including cerebral palsy in children and movement disorders (head, neck, shoulder and face). Dr Georghy’s experience with BT is since 1995. He observed no major complications.

As with all medicines, Botulinum Toxinum may cause some side effects. Generally, any side effects experienced with Botulinum Toxinum are temporary.

After the first treatment, the effects of BT last an average of 6 - 7.5 months. However the amount of time BT remains effective may vary. When your symptoms begin to return, see your doctor.

 

Fat Grafting

Fat grafting is a procedure using fat taken from unwanted fatty deposits elsewhere in your body and relocating them.

Our fat grafting procedure is performed under local anaesthetic in our clinic. Tiny parcels of fat are harvested from a donor area such as your thighs, inner knees or lower abdomen (or flanks in men) through a fine specialised cannula. The fat is condensed using a centrifuge and then injected into other fat deficient areas of your body through puncture holes without cutting or removing skin.

The fat can be injected into different parts of the body, but is most commonly used to re-volume the face, neck and hands. The procedure uses your own fat cells which reduces the chances of allergy or contamination.

Large Pores

Pores are essential to your skin's health by providing oil to keep skin soft and hydrated. However, you don't necessarily want to be reminded of their existence every time you look in the mirror. Pores are more prominent when they are dilated, infected, or clogged with oil, making them appear larger than they really are. While it isn't possible to permanently "shrink" large pores, there are a number of ways to make them less noticeable.

Causes of Large Pores

There are two main factors that contribute to large pores: genetics and age. People who have skin that is naturally thick and oily tend to have larger pores. As people get older, too, sun damage and decreased skin elasticity can cause pores to dilate. The skin thickens, causing tiny cells to collect around the edge of the pores and make them look larger. Blackheads are also a big contributor to large pores. When the pore becomes clogged by dirt and bacteria, oil within the pore begins to accumulate, causing the pore's diameter to expand.

There are a number of factors that will cause large pores, but of the many, there are three main culprits.

  •  The first one is age. As we age we will naturally lose the elasticity in the surrounding skin that supports the pores, causing pores to appear bigger than they actually are.
  • Secondly, men are more likely to have larger pores compared to women from their genetic makeup and increased oil production.
  • And last but not least, our body uses a substance called Sebum, which naturally hydrates our skin to prevent from drying. This substance however is very oily and individuals who produce more than average will find that oily skin causes larger pores.
  • Other causes: Sun damage, blackheads.

Microneedling (MicroPen & DermaPen)

Dr Georghy has been applying MicroNeedling concept since early 2000. The dermal roller was the first system and was reinvented by Dr Fernandez a plastic surgeon from South Africa.

Automated Micro-Needling (also known as Collagen Induction Therapy or CIT) is a new innovation in aesthetic medicine for the treatment of fine lines, acne scars and improvement of the skin’s texture, tone and colour. Moles

There are several skin lesions that are very common and benign (non-cancerous). These conditions include moles, freckles, skin tags, benign lentiginous, and seborrheic keratosis.

Moles and Your Skin

Moles are growths on the skin that are usually brown or black. Moles can appear anywhere on the skin, alone or in groups. Some people are prone to develop moles.

Most moles appear in early childhood and during the first 25 years of a person's life. It is normal to have between 10-40 moles by adulthood.

What Causes a Mole?

Moles occur when cells in the skin grow in a cluster instead of being spread throughout the skin. These cells are called melanocytes, and they make the pigment that gives skin its natural colour. Moles may darken after exposure to the sun, during the teen years, and during pregnancy.

How Do I Know if a Mole Is Cancer?

If you notice changes in a mole's color, height, size, or shape, you should have a doctor evaluate it. You also should have moles checked if they bleed, ooze, itch, or become tender or painful and they change color.

The following ABCDEs are important characteristics to consider when examining moles. If a mole displays any of the signs listed below, have it checked immediately by a dermatologist. It could be cancerous.

                             The acronym: ABCDE

  • Asymmetry. One half of the mole does not match the other half.
  • Border. The border or edges of the mole are ragged, blurred, or irregular.
  • Color. The color of the mole is not the same throughout or has shades of tan, brown, black, blue, white, or red.
  • Diameter. The diameter of a mole is larger than the eraser of a pencil.
  • Elevation/Evolution. The mole is changing in size, shape, or color.

What are the different types of moles?

Congenital nevi are moles that appear at birth.

Dysplastic nevi: They tend to have uneven color with dark brown centers and lighter, uneven edges. These moles tend to be hereditary. People with dysplastic nevi might have more than 100 moles and have a greater chance of developing malignant (cancerous) melanoma. Any changes in the mole should be checked by an experienced doctor to detect skin cancer.

How are moles treated?

If Dr Georghy believes the mole needs to be evaluated further or removed entirely, he will first take a biopsy (small tissue sample of the mole) to examine thin sections of the tissue under a microscope. The treatment is based on the histopathology result.

Scars & Keloids

INTRODUCTION    

Healing is the inevitable consequence of the Acute Inflammatory Response, once the injurious agent(s) has been removed and necrotic material is demolished. 

 Healing can take one of three forms:

  1. Resolution: occurs when there has been no cell death, involves removal of inflammatory exudate 
  2. Regeneration of cells occurs after cell death. For this to occur, cells must be able to replicate and the anchoring structure (basement membrane) to be intact
  3. Fibrous Repair: the process where a wound is replaced by a scar; involves granulation tissue, cell proliferation, collagen synthesis, scar contracture and scar remodelling.

In processes 1 and 2, normal structure and function return. In process 3 specialised tissue (such as cardiac muscle, neurones) is replaced by connective tissue. In reality, healing is probably a variable mixture of all the above, depending on the tissue. 

The healing process is best exemplified by healing of skin wounds (where the epidermis regenerates, whilst the dermis undergoes fibrosis).

Skin Imperfections

The skin is the biggest organ of the human body and probably the most complex. The skin has not only protective properties but also acts as “external brain” or antenna to the human body.

Although you may not typically think of the skin as an organ, it is in fact made of tissues that work together as a single structure to perform unique and critical functions. The deeper layer of skin is well vascularized (has numerous blood vessels). It also has numerous sensory, and autonomic and sympathetic nerve fibers ensuring communication to and from the brain.

The skin protects the rest of the body from the basic elements of nature such as wind, water, and UV sunlight. It acts as a protective barrier against water loss, due to the presence of layers of keratin and glycolipids in the stratum corneum. It also is the first line of defense against abrasive activity due to contact with grit, microbes, or harmful chemicals. Sweat excreted from sweat glands deters microbes from over-colonizing the skin surface by generating dermicidin, which has antibiotic properties.

Skin Pigmentation

The pigmentation of one’s skin is the result of millions of years of evolution, which explains the wide range of skin tones we see in everyday life. People with origin close to the Equator, where the sun is the most intense, had to develop protective mechanism against dangerous UV rays.
The result? → An increase in the production of melanin, a natural skin and hair colorant that absorbs UV radiation and protects the body from damage. In fact, the epidermis of a dark-skinned person absorbs up to 25% more UV than that of a light-skinned counterpart.

How do I prevent unbalanced pigmentation?

  • Pigmentation disorders are caused by a myriad of factors, which include sun damage, hormonal imbalance, and skin trauma or disease. Of all these causes, sun damage is both the most common, and the most easily prevented! Simply applying a sunscreen with a minimum SPF15 every day, and avoiding peak sun times around midday, will do a lot to prevent the sun damage that can make your skin’s pigment act up. Jan Marini- Daily Face Protectant (SPF30) provides optimum daily sun protection.
  • Hormonal imbalance, a cause of pigmentation mostly affecting women, can occur during any time that the body’s natural hormone balance is disturbed. These triggers can include hormonal changes, such as those that happen during the menstrual cycle, menopause, prolonged stress, pregnancy or even when taking birth control pills. Unlike pigmentation related to trauma or sun damage, hormonally-induced hyperpigmentation generally disappears when balance is restored to the body, and it can be treated during the interim period with topical skin brighteners.
  • The pigmentation related to skin trauma (cuts, wounds, scrapes and the like) is difficult to prevent in the case of accidental damage to the skin. Picking at acne lesions is also, of course, a big no-no, and will only exacerbate the pigmentation associated with the wound healing.
  • How much you can reduce the appearance of hyperpigmentation depends largely on what is causing the problem, as well as how deeply into the skin the pigmentation extends.

Vampire Facial (The Kim Kardashian Facial)

Vitiligo

Definition: Vitiligo is a skin disorder that causes depigmentation from areas of the skin resulting in white spots or patches. It occurs when melanocytes, the cells responsible for skin pigmentation, are “poisoned” and die or are unable to function and produce melanin.
Despite research, the cause of vitiligo is unknown.

Vitiligo (white patches) is notoriously difficult to treat, however, thanks to years of experience and passionate follow up Dr Joseph Georghy has developed a treatment for Vitiligo – one of the most extreme examples of hypopigmentation. Having observed that vitiligo may be associated with a deficiency in vitamin D (or the ‘sunshine vitamin’), Iron deficiency, thyroid disease, hormonal imbalance, autoimmune disorders, etc., Dr Georghy uses combine therapy to  bio-stimulate  the melanogenesis (natural process of melanin biosynthesis) and possibly reactivation of the patient's ‘inactive’ or ‘sick’ melanocytes. This treatment results in melanin producing melanocytes, negating the need for ongoing prescription medication and avoiding their associated side effects.
